Acknowledgement of Risk I understand that participation in yoga, meditation, physical movement, breathwork, wellness retreats, and/or private sessions may involve inherent risks, including but not limited to physical injury, emotional discomfort, illness, or property damage. I voluntarily choose to participate in these activities with full knowledge of the risks involved. 2. Health, Pregnancy & Medical Disclosure I affirm that I am physically and mentally able to participate in yoga and wellness-related activities. If I am pregnant, become pregnant, or am postnatal, I will consult with my physician before participating and understand that I do so at my own risk. I acknowledge that modifications and precautions may be necessary and that it is my responsibility to communicate any health concerns or limitations to the instructor before class begins. 3. COVID-19 Disclaimer I understand that attending in-person classes, workshops, or retreats may expose me to the risk of contracting COVID-19 or other communicable illnesses. I voluntarily assume this risk and agree that KJ Flows, its instructors, and associated venues are not responsible for any exposure, illness, or related complications resulting from my participation. 4. Release of Liability In consideration of being permitted to participate in any offering hosted by KJ Flows, I release and discharge KJ Flows, its owner (Katie Jordan), instructors, staff, collaborators, volunteers, and venue partners from any and all liability, claims, demands, or causes of action arising from any injuries, damages, or losses sustained while participating — whether caused by negligence or otherwise. 5. Venue & Environment Acknowledgement I understand that sessions may occur in indoor or outdoor settings, public or private venues, or during domestic/international travel. I accept full responsibility for my participation in these varied environments and waive any liability related to environmental hazards, terrain, weather, wildlife, or unfamiliar conditions. 6. Personal Belongings I understand that I am responsible for my personal belongings and valuables during all KJ Flows classes, events, or retreats. KJ Flows and its affiliates are not liable for any lost, stolen, or damaged items. 7. Emergency Medical Care In case of emergency, I authorize KJ Flows and its representatives to obtain necessary medical treatment on my behalf and agree to bear any financial responsibility for such care. 8. Photo/Video Release I grant KJ Flows permission to use any photographs, video, or media captured during classes, events, or retreats for promotional or educational purposes. I understand that I may opt out by submitting a written request prior to the event. 9. Cancellation & Refund Policy I understand and agree to the cancellation and refund policy for each offering. For retreats, deposits are typically non-refundable and final balances must be paid by specified due dates. I acknowledge that travel insurance is strongly encouraged for multi-day retreats. 10. Governing Law & Jurisdiction This agreement shall be governed by the laws of the State of Florida, and any disputes arising hereunder shall be resolved exclusively in the courts located in Miami-Dade County, Florida. 11. Agreement to Terms By checking the box on this form, submitting payment, or participating in any KJ Flows offering, I confirm that I have read and understood this Waiver and Release of Liability in full and agree to abide by its terms voluntarily and knowingly.
